I am Puerto Rican — born and raised on an island that shaped everything: who I am and my values.
As a student at the University of Puerto Rico, I knew psychology was my path and trauma was my calling. But I also understood something about the profession I was choosing: the most specialized doctoral programs in evidence-based trauma treatment were in the United States. That is not a reflection of the talent or the people of Puerto Rico. It is a reflection of the structural inequities that have long determined which communities get to build their own research and training pipelines — and which ones are left out.
Like so many Puerto Ricans who seek doctoral training, I left. It wasn't easy. But I carried the island with me, and always knew I would find my way back.

In September 2017, Hurricane María devastated Puerto Rico. I watched as my island — my people, my family, everything that made me who I am — faced one of the worst disasters in its history. The trauma was collective and immediate. Everyone was impacted — children, adolescents, and adults.
I returned to the island to train community agencies and organizations in Psychological First Aid (PFA). Together, we asked: how do we take what we know helps with collective trauma after disasters and adapt it to the reality of Puerto Ricans — their language, their culture, their way of understanding the world?
Nearly a decade after María, the answer is: yes, it can be done, and we are doing it together.
Over the past decade, I have had the privilege of training clinicians and strengthening access to trauma-informed, evidence-based services in Puerto Rico. I have brought this same work to countries such as El Salvador, Honduras, and Colombia, and across the United States.
During that time, my professional home was academia: research, clinical training, and mentoring the next generation of trauma specialists. I am deeply proud of this work. But for some time I have felt a call toward something closer — to reach further, to put what I know and who I am in service of people, beyond the classroom and conference room.
Visión Plena was born from that call. It is where everything converges: therapeutic services for children and adults, training and consulting for the clinicians who accompany them, mentorship for those beginning this path, and a genuine commitment to making evidence-based trauma treatment truly accessible — in Spanish and English, through telehealth, for the communities that need it most.

My goal with children and adolescents is for them to feel safe, that they matter, and that there is hope after the storm. I use the most scientifically supported treatment for trauma in minors — in an active, engaging, and age-appropriate way.
As a caregiver, you will be an essential part of the process from the beginning. You will learn tools to support your child at home, manage behavioral challenges, and strengthen communication between you. You don't have to do this alone. I'm here so that your child — and you — can move forward.

I love to travel, be in nature, and spend time with my husband and family. Silence recharges me. I am naturally curious; I learn from everything and everyone, and I enjoy a good conversation — even better when it comes with an espresso. Visión Plena offers therapy to children, adolescents, and adults experiencing the effects of trauma — whether it happened recently or long ago. You may have suffered a single traumatic event, complex trauma, or the cumulative effects of adversity, discrimination, or disaster.
Our therapy is available in English and Spanish. Dr. Orengo-Aguayo is based in Katy, Texas, and sees all her clients virtually, allowing you to access services from the convenience and privacy of your home. Limited in-person therapy spots may be available for clients living in the Katy/Houston, Texas area.

Trauma is something that happened to you — or that you witnessed — that was so difficult or dangerous that it still affects how you feel, how you think, or how you live day to day. It can be a single experience or many things that happened over time. And it doesn't have to have happened directly to you — sometimes what happens to the people we love also affects us deeply.
Examples: physical or sexual abuse · domestic or partner violence · community violence or shootings · natural disasters · serious accidents · going through something very painful or frightening in the hospital · losing someone you love suddenly or violently · having to flee your country or a dangerous place · experiencing racism or discrimination
Trauma does not look the same for every person. It can show up as anxiety, sadness, anger, or changes in behavior. It can affect how you sleep, how you feel in your body, or in your relationships with others. Some people remember what happened clearly. Others don't — they just know something is wrong. In children, trauma often shows up in behavior: difficulties at school, changes in relationships with caregivers or peers, or reactions that are hard to understand.

Treatment at Visión Plena is grounded in evidence-based, trauma-focused approaches. We identify the best approach for each person based on their age, history, and goals — always with compassion, respect for their values and culture, in English or Spanish, via telehealth. Our goal is simple: to help you achieve a full vision where trauma no longer darkens your life and light can enter again.
TF-CBT is one of the most extensively studied and well-supported treatments for trauma in children and adolescents. It has been researched across many countries and consistently demonstrates strong outcomes. It is a short-term therapy that helps children and adolescents overcome trauma — including symptoms such as sadness, fear, anxiety, posttraumatic stress, and behavioral changes.
Treatment includes individual sessions with the child or adolescent and separate sessions with the caregiver — both learning the same important skills to address trauma-related symptoms and behaviors. As therapy progresses, youth and caregivers also meet together (conjoint sessions). TF-CBT has strong evidence across a wide range of traumatic experiences in children ages 3 to 18 and is available in Spanish and English.
Dr. Orengo-Aguayo is a nationally certified TF-CBT therapist and trainer and has trained more than 100 therapists in the United States, Puerto Rico, and Central America. At Visión Plena, she puts all that experience directly in service of her clients.
Age note: TF-CBT is effective for children starting at age 3. For ages 3–6, in-person care is recommended. Since Visión Plena is a telehealth-only practice, we currently offer TF-CBT for children ages 7 and up. If your child is younger, we are happy to help connect you with an in-person provider — or you can find a certified TF-CBT therapist near you ↗. We offer evidence-based treatments adapted to the needs of each person. PE, CPT, and WET are treatments designed specifically for Post-Traumatic Stress Disorder (PTSD), while ACT is a flexible approach that can help with a variety of emotional difficulties and life experiences.
PE is one of the most effective treatments for PTSD in adults. It helps people gradually and safely approach trauma-related memories, emotions, and situations they have been avoiding. With practice, these memories and situations become less overwhelming and the person can re-engage in important activities in their life.

CPT is an effective treatment for PTSD that helps identify how trauma has affected the way we think about ourselves, others, and the world. It helps recognize and challenge thoughts that keep us stuck — called "stuck points" — especially around safety, trust, power and control, self-esteem, and intimacy.

WET is a brief five-session treatment for PTSD. It uses guided writing about traumatic experiences to help process memories, thoughts, and emotions related to what happened. It requires no between-session assignments and can be a good option for people who prefer a brief, structured treatment.

It also helps identify what truly matters — our values — and take steps toward a life with greater purpose and meaning. Yes. Sessions are conducted through an encrypted, HIPAA-compliant video platform — your information is protected and sessions are never recorded without your consent. Your therapist also connects from a private space. Using headphones helps give you more privacy.

Yes. Studies show that TF-CBT via telehealth works with school-age children and adolescents. Also, not having to leave home makes it easier for families to keep appointments.
That said, every child is different. Your therapist will work with you to make sure telehealth is the best option.

Telehealth is a good option for most adults, adolescents, and school-age children seeking therapy for trauma. However, it may not be appropriate when someone is experiencing an acute mental health crisis or imminent safety concerns, requires a higher level of care, cannot participate safely and privately from their location, or if your child is under 7 and has difficulty staying engaged in front of a screen for an extended time.

If you are not in crisis, a free 15-minute consultation is the ideal place to start. We will talk about your particular situation and together determine whether telehealth is the best option for you — and if it isn't, we will help you find the most appropriate care.
Your environment matters. To get the most out of each session:
— Find a private space where you can close the door. A desk and chair are recommended. Sitting or lying in a bed or connecting from a vehicle is not allowed.
— Use headphones (crucial for privacy and sound quality)
— Silence your phone and close unnecessary tabs
— Position your camera at eye level, with light in front of you (not behind)
— Have water nearby and something that helps you feel grounded
— Connect a few minutes early to get settled
— Test your camera and audio before starting
— Dress as if you were going to an in-person appointment
If you can't find a private space at home, talk with your therapist to identify private and safe alternatives.
You need a computer, laptop, or tablet with a camera, microphone, and a stable internet connection. We recommend a laptop or desktop — the larger screen makes eye contact easier and facilitates the use of interactive materials during sessions.
A smartphone can work if you have no other option, but it's not ideal. In all cases, headphones are recommended to improve audio quality and protect your privacy.
Technology sometimes fails. Before beginning, you and your therapist will create a simple plan for those moments: what to do if the call drops, how to reconnect, and what will happen in case of an emergency.
What matters is where you are physically located during each session — not your residential address. Dr. Orengo-Aguayo holds active licenses in Texas, South Carolina, Louisiana, and Puerto Rico, and is authorized through PSYPACT ↗ to serve clients in PSYPACT participating jurisdictions. Check the current PSYPACT map to confirm your state.
Telehealth sessions are not available while traveling internationally. Your location will be confirmed at the start of each session.

We begin with an initial 90-minute session — a space to get to know you better, hear your story and goals, and together create the best treatment plan for you. Then, we typically meet weekly for 50-minute sessions.
Total time in therapy depends on each person and their situation. Generally, trauma-focused therapies last between 3 and 6 months, though sometimes it may be a bit less or more.

Yes. Every new client starts with a free 15-minute phone or video consultation — at no cost. This is an opportunity to share what brings you here, ask questions, and see if Visión Plena feels like the right fit before committing.
Treatment length depends on the approach and your needs. TF-CBT typically lasts 12 to 25 sessions; PE and CPT are shorter, structured protocols of 8 to 15 sessions; WET is the briefest at just 5 sessions. ACT is more flexible and adapts to each person.
Sessions are 50 minutes and generally weekly. We always begin with a 90-minute initial session to get to know each other, understand your story, and create the best treatment plan for you. During your free consultation we will talk more about what to expect.
Therapy services are only available to clients located in states or territories where Dr. Orengo-Aguayo is licensed or authorized through PSYPACT. Currently these include Texas, Louisiana, South Carolina, Puerto Rico, and all states that are part of PSYPACT. Who it's for
Requirements for clinicians (in the US/Territories): Must have a master's or doctoral degree in a mental health discipline, be licensed, and ideally have prior experience working with children and adolescents. Note: For clinicians in Central or South America, they must meet their country's requirements to offer mental health services.

Before applying to a training cohort, an excellent first step is to complete TF-CBT Web 2.0 — a self-paced online course that introduces the TF-CBT model. It takes approximately 11 hours to complete, offers continuing education credits, and is available in English and Spanish for a one-time fee of $35.

The story of why I became a psychologist specializing in trauma begins on an island.
I grew up in Puerto Rico surrounded by family, warmth, and community — and with an early awareness that people who were suffering did not always have access to the help they needed. When I entered the psychology program at the University of Puerto Rico, Río Piedras Campus, I knew I had found my path. What I did not yet know was how far it would take me.
Like many Puerto Ricans seeking specialized doctoral training in clinical psychology, I left. It was not easy. Puerto Rico is my home — its people, its history, its rhythms are part of me in ways that distance never erases. But I left because the training I sought was not available on the island. I completed my master's and doctorate in clinical psychology at the University of Iowa, and later completed a postdoctoral fellowship focused on trauma research and treatment with children, adolescents, and adults — funded by the National Institute of Mental Health (NIMH).
During my doctoral training, I conducted a study with people who were incarcerated. What I found was striking: every single participant had experienced multiple traumatic events before the age of eighteen. Every one of them. That finding changed the course of my career. It made me understand that if we could intervene earlier — if we could reach children and families sooner — we could change trajectories. Not just individuals, but entire communities.
That conviction has guided everything I have done since.
In September 2017, Hurricane María devastated Puerto Rico. In the weeks that followed, as communities across the island struggled to survive and rebuild, I joined a collaborative effort with local organizations to understand and respond to what children and families were experiencing.
What emerged was one of the largest post-disaster mental health screenings in history — reaching more than 96,000 students and more than 8,000 teachers across the island. The data confirmed what communities already knew: children were suffering, and the mental health workforce needed more tools, more training, and more support to respond to an unprecedented scale of trauma.
I did not arrive as an outsider with solutions. I arrived as someone with deep roots in my island and a specific set of skills to offer. Together with Puerto Rican mental health professionals, community leaders, schools, and families, we built something real. We trained first responders in Psychological First Aid (PFA), an evidence-informed intervention designed for the days and weeks immediately following a disaster. Then we offered Skills for Psychological Recovery (SPR), the next step for those who were still struggling with the effects of the disaster but did not yet need specialized therapy services. Finally, we implemented Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) — the most effective trauma treatment available for children and adolescents — in community mental health clinics and schools across the island. We trained clinicians. We measured outcomes. We published findings. Together, we contributed to the evidence supporting the use of TF-CBT in Puerto Rico.

That work continued. With federal funding through SAMHSA, I directed the Puerto Rican Center for Trauma Training and Research, one of the few federally funded centers dedicated to building trauma-focused workforce capacity in Puerto Rico. Our team has trained more than 10,000 individuals in evidence-based trauma interventions and disaster response. A randomized controlled trial evaluating the effectiveness of TF-CBT via telehealth versus in-person delivery in community clinics across the island is currently underway.
Recognizing that the challenges facing Puerto Rico — access to specialized training, geographic barriers, language barriers, limited resources — are replicated in communities across Latin America, we extended this work to El Salvador, Honduras, and Colombia. In those contexts as well, the goal was never to import solutions, but to adapt and build alongside the professionals and communities who do this work every day.
I co-authored the first telemental health manual published in Spanish — because geography and language should never determine who has access to evidence-based mental health care.
During this time, my professional home has been academia: research, clinical training, and mentoring the next generation of trauma specialists. I am deeply proud of that work. But for some time I have felt a call toward something closer — to reach further, to put what I know and who I am in service of people, beyond the classroom and conference room.
Visión Plena was born from that call. It is where everything converges: therapeutic services for children and adults, training and consulting for the clinicians who accompany them, mentorship for those beginning this path, and a genuine commitment to making evidence-based trauma treatment truly accessible — in Spanish and English, through telehealth, for the communities that need it most.
The name says it all: visión plena — full vision. The idea that trauma clouds what we can see — in ourselves, in our relationships, in our future — and that healing is about moving those clouds until the light comes through. At Visión Plena, we do this together. We heal and learn in community.
